Clarion Partners Acquires 103,000 SF PAM Health Rehabilitation Portfolio in Colorado and Florida via Colliers
NEW YORK — Clarion Partners, a specialist investment manager affiliated with Franklin Templeton, has completed the acquisition of the PAM Health IRF Portfolio, a 103,000-square-foot, two-property collection of purpose-built inpatient rehabilitation hospitals fully leased to PAM Health. Colliers announced the transaction on March 18, 2026.
The Colliers U.S. Healthcare Capital Markets team represented the seller, an affiliate of MedProperties Realty Advisors, in the deal. Financial terms were not disclosed.
Portfolio Overview: Colorado and Florida Assets
The portfolio comprises two recently delivered, Joint Commission-certified facilities, each featuring 42 licensed beds and 100% occupancy under long-term leases to PAM Health, a national provider of post-acute care services.
PAM Health Rehabilitation Hospital of Greeley, located at 6810 W. 10th Street in Greeley, Colorado, was completed in early 2024. The facility offers comprehensive care programs including stroke, orthopedic, brain injury, and pulmonary rehabilitation, along with on-site dialysis and bariatric rooms. The hospital serves the rapidly growing population of northern Colorado.
PAM Health Rehabilitation Hospital of Venice, situated in Nokomis, Florida within the Sarasota–Venice corridor, was delivered in 2023. The facility provides specialized neurological, cardiopulmonary, orthopedic, and complex medical care, and features outdoor therapy areas, dialysis services, and advanced rehabilitation equipment. The property serves the expanding Sarasota–Venice market.
Colliers Team
The Colliers brokerage team on the transaction was led by Senior Vice President CJ Kodani, Executive Vice President Jordan Selbiger, Director Josiah Gunter, and Senior Director David Hochstrasser, all members of the firm's U.S. Healthcare Capital Markets group.
Kodani cited investor appetite for healthcare real estate assets as a driver of the transaction. "Inpatient rehabilitation hospitals continue to attract significant institutional capital because they deliver exactly what investors are seeking — stable income, strong cash flow, and built-in inflation protection," Kodani said. "These purpose-built facilities, combined with long-term lease structures and a nationally recognized operator, resonated strongly with both investors and the lending community."
Darryl Freling of MedProperties Realty Advisors also commented on the sale. "This transaction reflects MedProperties' continued commitment to identifying institutional-quality healthcare real estate that creates long-term value for our investors," Freling said. Institutional Interest in Healthcare Assets
The portfolio transaction highlights continued institutional interest in healthcare-focused commercial real estate, particularly inpatient rehabilitation facilities backed by established operators under long-term leases. Both properties are described as mission-critical healthcare infrastructure anchored by one of the nation's most experienced post-acute care operators.
Together, the properties represent a combined 84 licensed beds across two regional markets — northern Colorado and the Sarasota–Venice corridor in Florida.
